I the undersigned parent/guardian of the above-named child, hereby agree and acknowledge the following:
Voluntary Participation: I confirm that my child is participating voluntarily in the activities hosted by Little Island Brewery Co. during the June 2025 school holidays.
Health Declaration: My child is in good health and physically able to participate in the listed activities. I will inform the organisers in writing of any allergies, dietary restrictions, or medical conditions.
Injury & Accident Waiver: I understand that while all reasonable care and supervision will be taken, participation involves inherent risks. I shall not hold Little Island Brewery Co., its staff, facilitators, or partners liable for any injury, loss, or damage sustained during participation.
Medical Consent: In case of emergency, I authorise the organisers to seek appropriate medical attention for my child. I agree to bear any medical expenses incurred.
Photo & Video Release: I give permission for my child’s photos/videos taken during the event to be used for promotional or marketing purposes by Little Island Brewery Co.
Drop-Off & Pick-Up Responsibility: I agree to drop off and pick up my child on time. I understand that Little Island Brewery Co. is not responsible for the child outside of the designated event time slots.
